In this daytime role, you will conduct assessments, develop and implement individualized treatment plans, document progress, and collaborate closely with physicians, nurses, and rehabilitation teams to deliver patient-centered care. You will educate patients and families, contribute to discharge planning, and participate in quality and process improvement.
