Licensed Clinical Social Worker

HarmonyCares is seeking a Licensed Clinical Social Worker to deliver compassionate, home-based behavioral health support to medically complex, home-limited patients. In this role, you will complete psychosocial assessments, provide evidence-based counseling, and coordinate community resources to address social determinants of health. Working as part of an interdisciplinary, physician-led team, you will help design care plans, support palliative and end-of-life care, and participate in case conferences to reduce avoidable hospitalizations.
