The Role:
The Community Supports Coordinator manages the member journey following admission, helping ensure members receive consistent, uninterrupted services. This role focuses on utilization and reauthorization management, proactive member and caregiver engagement, and coordination with funding partners and health plans to support appropriate use of authorized services.
Primary Responsibilities
- Monitor member utilization and authorized hours each billing cycle, proactively addressing underutilization, overages, and potential service disruptions.
- Manage utilization and reauthorization cases in Salesforce, ensuring required documentation is collected and extensions are completed in a timely manner.
- Partner with funding sources, health plans, and Case Managers to communicate member status, resolve authorization issues, and support continuity of services.
- Build and maintain relationships with members and caregivers, providing proactive outreach, resources, and support throughout ongoing service delivery.
- Respond to member and caregiver inquiries across phone, chat, text, and email, working to provide timely and effective resolution.
- Maintain accurate member records, including service utilization, authorization status, interactions, and required health plan updates.
- Collaborate across internal teams and external partners to resolve service issues, meet closed-loop referral and care integration requirements, and maintain HIPAA compliance.
This is a hybrid position, coming into the El Segundo office 1x per month.
Please note that this is a TEMPORARY position.
What You Bring to the Table:
Qualifications
- High school diploma or GED required; bachelor’s degree preferred.
- 1–3+ years of customer service or other service-oriented experience.
- Experience with ticket management systems and processes.
- Proficiency with Microsoft Office, including Word, Excel, and Outlook, and experience with Salesforce.
- Ability to manage a high volume of communications and cases while meeting deadlines in a fast-paced environment.
- Bilingual Spanish preferred.
- Reliable internet service required.
