Description Northwestern Medicine has an exciting opportunity for a Physician Assistant or Acute Care Nurse Practitioner to work on our Chronic Obstructive Pulmonary Disease (COPD) team. This Advanced Practice Provider will be working in a hybrid setting (inpatient and outpatient clinic). Support inpatient pulmonary consults and outpatient pulmonology clinic services across the continuum of care. Demonstrate a strong interest in COPD management and the delivery of evidence-based pulmonary care. Provide timely post-discharge follow-up and ongoing patient management to help reduce hospital readmissions and improve outcomes.
