- Assist patients with activities of daily living (ADLs) including bathing, grooming, dressing, toileting, and mobility.
- Provide light housekeeping, meal preparation, and companionship in accordance with the patient's plan of care.
- Monitor and report changes in the patient's condition, behavior, or environment to the supervising nurse.
- Follow physician orders and the individualized care plan under RN supervision.
- Accurately document care provided and complete Electronic Visit Verification (EVV) for each visit.
- Maintain a safe, clean, and supportive environment while respecting patient privacy and dignity.
- Home Health Aide License/CNA License is required
- High School Diploma or GED
- One year prior professional experience
- Driver’s license required
- CPR certification required
- Ability to pass a criminal background check, TB screening, and required health clearances.
- Strong communication skills, dependability, and a genuine commitment to patient well-being.
