Martin Luther King, Jr. Community Hospital seeks an RN Coordinator for Enhanced Care Management (ECM) to support high-risk, underserved patients in South Los Angeles. The RN Coordinator leads comprehensive assessments, develops individualized care plans, and coordinates services across inpatient, outpatient, and community settings. This role collaborates closely with physicians, social workers, and community partners to address medical, behavioral, and social needs, reduce readmissions, and close care gaps. You will provide patient education, track outcomes, and advance MLKCH’s mission of health equity and culturally responsive, community-centered care.
