This Full-time Opportunity will work 80 hours bi-weekly from 8am - 4:30pm. Additional pay for shift differential. Summary:

This position is responsible for the evaluation and disposition planning needs of patients admitted to Behavioral Health Unit (BHU) and will:

• Complete psychosocial assessments with a focus on patients' requirements as they transition to the next level of care
• Assess, plan, and coordinate patients' post-acute health care services
• Participate in Interdisciplinary Treatment Rounds and other patient care conferences
• Shares the responsibility of conducting Involuntary Admission Hearings in the role of Hospital Presenter
• Collaborate with psychiatric providers, nurses, other social workers / therapists, to foster a coordinated approach to discharge planning
• Accurately complete required documentations and in a timely manner
• Coordinates with patients' outpatient care providers for needed services and resources as needed
• Communicates with Utilization Review Staff on any denials, issues or barriers to discharges
• Obtain needed authorization for needed outpatient services
• Connect patients to community providers including psychiatrists and therapists to make sure that patients have access to prescribed medications
• Assist patients to schedule medical appointments to include transportation resources

Qualifications:

• Empathetic toward individuals with complex health needs and their families
• Strong communication, writing, and customer service skills. Knowledge of community resources
• Ability to work in a fast-paced environment while managing multiple priorities and maintaining professionalism
• Excellent time management skills and ability to problem solve and exercise good judgment

Education:

Master of Social Work or related field (REQUIRED)

Licensure and Certification:

LCSW-C (PREFERRED)

LMSW, LGPC (REQUIRED)

BLS/CPR Certification required
