---- Job Details: General Notes Dell Medical School is seeking a Behavioral Health Care Coordinator Team Lead. This is a grant funded position with an end date of 8/31/2027 that is renewable for another year based upon work performance and progress toward research goals. Purpose This role supports mental health programs and initiatives developed through the Texas Child Mental Health (TCMH) Care Consortium, as well as programs through the Behavioral Health Service Line at Dell Children’s Medical Center and Ascension Medical Group, with a focus on improving the quality and accessibility of mental health care for children and adolescents across Texas. The Behavioral Health Care Coordinator Team Lead plays a vital role in supporting children and families through both Dell Children’s Behavioral Health programs and TCMH programs like the Texas Child Health Access Through Telemedicine (TCHATT), a statewide initiative that partners with school districts to provide telemedicine and telehealth services aimed at identifying and assessing the behavioral health needs of children and adolescents, and connecting them to appropriate mental health resources. Reporting to the Behavioral Health Support Supervisor, this role leads mental health care coordination-and collaborates with clinical operations support staff. This position helps children and families navigate through the TCHATT program (or other TCMH programs), assists in connecting to providers in the community for long-term care if needed, and maintains confidentiality of information in all aspects of care. The Behavioral Health Care Coordinator Team Lead provides operational leadership by overseeing the work of other coordinators and guiding projects and workflows. Responsibilities Care Coordination and Clinical Collaboration- Connects patients and families to appropriate mental health and community resources for long-term care and provides ongoing support to ensure effective transitions.
- Coordinates with professionals, clinical support staff, schools, and families to reduce barriers and improve satisfaction.
- Maintains confidentiality in all aspects of care and documentation.
- Collaborates routinely with pediatric psychiatrists, psychologists, licensed counselors and operational staff to assess and address long-term psychological needs.
- Assembles clinical background and referral needs for individual patients and documents interactions in accordance with professional ethics and regulatory standards.
- Serves as the lead care coordinator for internal clinical and consultation tracks.
- Collaborates with supervisor and colleagues to develop and implement needed program development and project implementation and process improvements, both internally and externally – such as creating and updating internal referral processes, identifying service gaps for specialized populations.
- Provides oversight and monitoring of projects and initiatives, assign work, and monitor capacity and completion
- Works with the supervisor to create and implement onboarding training for new Care Coordinators and other employees.
- Hires, trains, and supervises graduate assistants, interns, or student learners as required by program leadership.
- Provides guidance in managing and educating on direct service referral pathways to support and advance program growth.
- Supports clinical staff on utilization of internal resources.
- Leads and manages community partnerships and referrals with identified agencies to ensure ongoing functioning of pathways.
- Collects and analyzes data for assigned programs or pathways.
- Prepares monthly reports, in collaboration with the leadership team, for submission to the Texas Child Mental Health Care Consortium.
- Ensures clinical documentation is available for reporting by monitoring compliance within secure data collection systems such as REDCap, Trayt, Athena, or others.
- Updates and supports EMR integration, including notes and orders.
- Implements documentation and technology-related special projects.
- Serves as a liaison for data and data-related projects as assigned.

MINIMUM QUALIFICATIONS:
Requires a Bachelor's Degree in Social Work, Psychology, Education, or a related field with at least 5 year(s) of experience in in a health care or educational setting (preferably K-12). - Proficiency in electronic systems including, but not limited to, Google Suite, Trayt, Slack, Athena or Epic, Calendly. Relevant education and experience may be substituted as appropriate. Preferred Qualifications Master's Degree in Social Work, Psychology, Education, or a related field with at least year(s) of experience in- Knowledge of managed care, medical terminology, patient referral process, claims, and CPT coding.
- Experience working with pediatric behavioral and mental health patient populations.
- Knowledge of educational practices supporting mental health and social emotional learning in K-12 settings.
- Experience developing, refining, and evaluating patient care plans or student education plans.
- Bilingual (English/Spanish) language proficiency.
