Job Description
Hours: 7:00P - 7:30A Responsibilities:
- Deliver patient care by incorporating the tenets of the professional practice model of Relationship Centered Care and through the application of the nursing process based upon theory, research, evidence-based practice, and approved organizational nursing standards.
- Assess and evaluate patient care needs and apply critical thinking skills in patient care management.
- Integrate relevant assessment and intervention skills in the delivery of nursing care.
- Collaborate with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care.
- Prioritize all aspects of patient care including teaching, rounding, coaching, and planning "after hospital care" and delegating to others as appropriate.
- Educate patients and caregivers while anticipating needs and readiness to learn, about their plan of care, transition of care, promotion of health, and prevention of disease.
- Communicate effectively and professionally with patients, family, and all members of the Interdisciplinary Patient Care Team.
- Manage assignments within the Care Delivery Model of Modified Primary Nursing and demonstrate allocation of material resources effectively.
- Role model the tenets of Relationship Centered Care through facilitation of actions such as Admission Welcome, Bedside shift Report, and Purposeful Hourly Rounding.
- Consider the patient's values, preferences, cultural diversity, expressed needs, and knowledge in all aspects of care.
- Establish and maintain a therapeutic relationship with the patient and family.
- Use evidence-based practices to increase understanding of patients' perceptions of care.
- Initiate service recovery for patients and family members who have concerns and escalate to Clinical Nurse Manager/Assistant Nurse Manager/Nursing Shift Manager or Patient Relations as needed.
- Demonstrate caring, respect, compassion, empathy, and active listening through dialogue, body language, and actions.
- Practice safe and efficient patient-centered care, identifying and escalating barriers and opportunities for improvement.
- Implement process improvement strategies based on institutional, evidence-based ensure and procedures.
- Collaborate with interdisciplinary teams to create and implement quality improvement projects, evidence-based practice activities, and nursing research studies.
- Utilize institutional information technology by documenting nursing practice to support quality and performance improvement initiatives.
- Must have a minimum of 2 years of neuro experience in a hospital setting.
- Must have a Bachelor of Science in Nursing (BSN).
- NYS RN License.
- Basic Life Support (BLS) certification.
- 2 years med surg/ambulatory care experience preferred.
- EPIC experience highly preferred.
- 1 year of travel experience for travel positions, not required for per diem positions.
