Mental Health Counselor - Partial Hospitalization Program - MSM - Full Time/Days

Description Under general supervision, serves as the primary intake and admissions coordinator for Adult Partial Hospitalization Program (PHP) and Intensive Outpatient Program (IOP) services, with approximately 0.5 FTE dedicated to each program. Reviews and coordinates referrals from inpatient units, emergency departments, ambulatory programs, community providers, and external referral sources. Perform clinical screenings, admission coordination, and treatment team collaboration to ensure appropriate level-of-care placement and successful program engagement. Provides milieu support and serves as a backup group facilitator when needed.
