Field Care Coordinator

Humana is seeking a Field Care Coordinator to support members in the community and at home, helping them navigate complex health and social needs. In this role, you will complete comprehensive assessments, create person-centered care plans, and coordinate medical, behavioral, and social services across providers and community resources. You will conduct in-person visits, provide bilingual support where needed, educate members and families, and monitor progress to reduce gaps in care. Working within an inclusive, member-focused culture, you will collaborate with an interdisciplinary team, document all activities in electronic systems, and ensure compliance with Humana and regulatory standards while positively impacting health outcomes.
