POSITION OVERVIEWThe RN Case Manager will provide telephonic care management and coordination for Dual Eligible Special Needs Plan (D-SNP) members, focusing on Transition of Care (TOC), clinical assessments, individualized care planning, member engagement, and closure of HEDIS/Medicare STAR quality gaps. The role requires a Registered Nurse with strong clinical case management skills, experience coordinating post-discharge care, and knowledge of Medicare Advantage and D-SNP requirements.ROLES & RESPONSIBILITIESConduct telephonic outreach to members and caregivers following hospital, SNF, rehabilitation, or emergency department discharge.Perform clinical, psychosocial, functional, and Social Determinants of Health (SDOH) assessments.Develop and update individualized care plans based on member needs and health risks.Coordinate care with providers, multidisciplinary teams, caregivers, and community organizations.Support Health Risk Assessments (HRAs), Individualized Care Plans (ICPs), and Interdisciplinary Care Team (ICT) activities..BENEFITSMedical | Dental | Vision | 401(k)
