<p> Job Title: Nurse Practitioner </p><p> Location: Barnabas Health Medical Group </p><p> Department Name: NBI Geriatrics </p><p> Req #: 0000257629</p><p> Status: Salaried </p><p> Shift: Day </p><p> Pay Range: $125,000.00 - $195,000.00 per year </p><p> Pay Transparency: </p><p> The above reflects the anticipated annual salary range for this position if hired to work in New Jersey. </p><p> The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience. </p><p> Requirements </p><ul><li> APN License </li><li> RN License </li><li> NP Certification </li><li> CDS and DEA </li><li> ACLS and BLS </li><li>2+ years of experience in geriatrics </li></ul><p> Shift: </p><ul><li> M-F (8:30a-4:30p)</li><li>4 clinic and consult days </li><li>1 day follow up home visits for high risk patients recently discharged from the hospital </li></ul><p> Essential Functions </p><ul><li> Provide comprehensive primary care with a focus on geriatric needs and chronic disease management </li><li> See a manageable caseload (about 8-12 patients per day), allowing time for thorough visits and care coordination </li><li> Work collaboratively with the team to support complex patients and promote healthy aging </li><li> Participate in hospital home visit initiative to help reduce readmissions </li></ul>
