OneHome seeks a Home Health COTA to provide compassionate, high-quality occupational therapy services in patients’ homes. Working under an OT, you’ll deliver treatments, support ADL training, and document progress in our technology-enabled platform. Collaborate with nurses, therapists, and physicians to execute care plans, prevent hospital readmissions, and promote safety and independence. You’ll educate patients and caregivers, coordinate equipment needs, and contribute to value-based care initiatives that improve outcomes and reduce costs in a mission-driven, patient-focused environment.
