Pulmonary Care Coordinator

Intermountain Health is seeking a Pulmonary Care Coordinator to support patients with respiratory conditions across our integrated care system. In this role, you will coordinate pulmonary care plans, schedule and track follow-up visits, and facilitate communication between physicians, respiratory therapists, nurses, and patients. You will provide patient education on respiratory therapies, inhaler and device use, oxygen safety, and lifestyle modifications, while monitoring outcomes and escalating concerns. You will also support data tracking, quality improvement initiatives, and adherence to clinical protocols in a collaborative, patient-centered environment.
