Humana is seeking an FCP RN Care Manager to support our Family Care Program, delivering telephonic and virtual care management to Medicare and Medicaid members. In this role, you will complete comprehensive RN assessments, create person-centered care plans, and coordinate services across physicians, behavioral health, pharmacy, and community resources. You will focus on chronic disease management, education, and early intervention to reduce avoidable ER visits and hospitalizations.
