Position Description:

Responsible for performing and documenting psychosocial assessments, coordinates educational and outreach services relevant to the social services, and provides liaison function between system-wide outpatient clinics.

Qualifications:

• Master's degree in Social Work from an accredited institution
• Experience in Palliative Care or hospice setting preferred
• Licensed Clinical Social Worker (LCSW) certification required
• Strong understanding of end-of-life care and ethical issues in Palliative Care.
• Minimum of 2 years' experience in a healthcare setting
• Familiarity with Epic, ICD-10, and CPT coding preferred.
