Family Nurse Practitioner / Physician Assistant (Wellness Provider)

Victory Recovery Partners is seeking a per-diem Family Nurse Practitioner or Physician Assistant to join its Wellness team in Farmingville, NY. This role provides medical services to patients enrolled in outpatient treatment programs, collaborating with an interdisciplinary team to coordinate comprehensive care.

Victory Recovery Partners offers Medication Assisted Treatment (MAT), Behavioral Health, Wellness, and Psychiatry services under one roof. This on-site position requires travel between multiple locations and is scheduled for Mondays from 8:00 AM to 8:00 PM.
ResponsibilitiesConduct comprehensive physical examinations as required by the program.Assess and treat acute and chronic medical conditions, including hypertension, diabetes, and thyroid disorders.Order and review laboratory tests, radiology studies, and EKG results.Administer and interpret PPD (tuberculosis skin) tests.Perform Hepatitis C testing, evaluation, and treatment.Educate patients on healthy habits, safe sex practices, STD prevention, smoking cessation, and lifestyle changes.Build positive patient relationships while promoting health and hygiene.Collaborate with interdisciplinary clinical teams to coordinate patient care.
Required QualificationsCompletion of an accredited Physician Assistant or Nurse Practitioner program.Valid, unrestricted New York State medical license.Valid Drug Enforcement Administration (DEA) certification.Ability and willingness to travel between multiple clinical locations.Strong communication and interpersonal skills to support patient education and coordinated care.
Compensation & Benefits
Per-diem Employees:Competitive hourly compensation of $65 per hour.Scheduled Mondays from 8:00 AM to 8:00 PM.Opportunity to work within an interdisciplinary outpatient healthcare team.Experience providing integrated medical and behavioral health services.
Work LocationFarmingville, NY – On-site, with travel to multiple locations required.
