In this field-based role, you will provide high-quality end-of-life care to patients in homes and facilities, focusing on comfort, dignity, and patient goals. Responsibilities include comprehensive assessments, symptom and pain management, medication administration, and coordination of interdisciplinary care. You will educate and support patients and families, document care in the EMR, and collaborate in a mission-driven, team-based environment dedicated to improving community health.
