Position OverviewWe are seeking an experienced Registered Nurse (RN) Care Manager for a hybrid position supporting older adults and individuals with complex healthcare needs. This role combines community-based field visits with remote care coordination, offering flexibility while making a meaningful impact on members’ health and quality of life.
ResponsibilitiesConduct in-home assessments to evaluate members’ clinical, functional, and social needsDevelop and manage individualized care plans in collaboration with physicians and interdisciplinary teamsCoordinate care across home care, hospitals, rehabilitation facilities, and long-term care settingsComplete approximately 6 home visits per week within the Bronx territoryMonitor member progress and adjust care plans to support optimal outcomesSupport safe discharge planning and help reduce hospital readmissionsCommunicate effectively with members, families, caregivers, and healthcare providersMaintain accurate and timely clinical documentation in accordance with applicable guidelinesAdvocate for members and connect them with appropriate healthcare and community resources
QualificationsActive, unrestricted New York RN licenseAssociate’s degree in nursing required; BSN preferredMinimum 3 years of RN experienceExperience in MLTC, home care, geriatrics, or case management preferredStrong communication, organizational, assessment, and critical-thinking skillsAbility to work independently in a hybrid environmentBilingual skills are a plus
