Medical Degree from an accredited college or university. </p><p>2. Diploma for completion of an American Board of Internal Medicine accredited Internal Medicine internship and residency. </p><p>3. NBME or Flex certification. </p><p>4. American Board of Internal Medicine certification or eligibility. </p><p>5. Unrestricted DEA number. </p><p>6.
