CarePartners is seeking a PRN Physical Therapist to provide home-based rehabilitation services to adult and geriatric patients across Connecticut. In this flexible role, you will perform comprehensive evaluations, develop individualized treatment plans, and deliver evidence-based interventions to improve mobility, safety, and functional independence. You’ll collaborate closely with nurses, therapists, and caregivers, educate patients and families, and maintain accurate, timely documentation.
