CenterWell Home Health is seeking a Home Health Social Worker to support seniors and adults with chronic conditions in their homes. You will conduct psychosocial assessments, develop person-centered care plans, and connect patients with community resources that promote safety, independence, and quality of life. Working within an interdisciplinary team, you’ll coordinate services, advocate for patient needs, and provide education to patients and caregivers.
