Hospital RN Case Manager

St. Mark's Hospital, part of the MountainStar Healthcare network, is seeking a compassionate RN Case Manager to join our Healthcare & Medical Services team on a PRN basis. In this role, you will coordinate and manage patient care plans, collaborate with interdisciplinary teams, support patient and family education, and ensure safe, timely transitions of care.
