- Position: Licensed Vocational Nurse (LVN)
- Employment Type: PRN / Part-Time
- Hours: 24–32 hours per week
- Pay Rate: $36.00–$40.00 per hour
- Location: Walnut Creek, CA
- Provide compassionate, high-quality nursing care to residents in accordance with individualized care plans.
- Administer medications and treatments as ordered and in accordance with facility policies.
- Monitor residents' conditions and promptly report changes to the appropriate healthcare professionals.
- Perform nursing assessments and document resident care accurately and timely.
- Assist with wound care, treatments, and other nursing procedures as directed.
- Monitor vital signs and other clinical indicators as appropriate.
- Communicate effectively with residents, families, physicians, and members of the interdisciplinary care team.
- Assist with admissions, transfers, and discharges as needed.
- Maintain accurate and complete nursing documentation.
- Follow all applicable federal, state, and facility regulations, policies, and procedures.
- Promote resident safety, dignity, independence, and quality of life.
- Current and active California LVN license in good standing.
- Current CPR certification preferred/required per facility policy.
- Previous skilled nursing or post-acute care experience preferred.
- Strong clinical assessment, communication, and organizational skills.
- Compassionate, dependable, and resident-focused.
- Ability to work effectively as part of an interdisciplinary healthcare team.
- Must be able to commit to 24–32 hours per week.
