- Provide direct patient care to senior clients in a home health setting on a PRN (as-needed) basis
- Administer medications, injections, and treatments as prescribed by the physician
- Monitor and document patient vital signs, health status, and progress
- Assist with wound care, catheter care, and other skilled nursing procedures
- Communicate effectively with patients, families, and the care team regarding patient needs and changes in condition
- Develop and follow individualized patient care plans in coordination with RNs and physicians
- Ensure accurate and timely clinical documentation in compliance with state and agency regulations
- Active Licensed Practical Nurse (LPN) license in the state of Virginia
- Prior home health, geriatric, or senior care experience preferred
- Strong clinical assessment and critical thinking skills
- Ability to work independently with minimal supervision in a home care environment
- Excellent communication and interpersonal skills with patients and families
- Valid driver's license and reliable transportation required
- CPR/BLS certification required or ability to obtain upon hire
