• Provide direct patient care to senior clients in a home health setting on a PRN (as-needed) basis
• Administer medications, injections, and treatments as prescribed by the physician
• Monitor and document patient vital signs, health status, and progress
• Assist with wound care, catheter care, and other skilled nursing procedures
• Communicate effectively with patients, families, and the care team regarding patient needs and changes in condition
• Develop and follow individualized patient care plans in coordination with RNs and physicians
• Ensure accurate and timely clinical documentation in compliance with state and agency regulations

• Active Licensed Practical Nurse (LPN) license in the state of Virginia
• Prior home health, geriatric, or senior care experience preferred
• Strong clinical assessment and critical thinking skills
• Ability to work independently with minimal supervision in a home care environment
• Excellent communication and interpersonal skills with patients and families
• Valid driver's license and reliable transportation required
• CPR/BLS certification required or ability to obtain upon hire
