Patient Navigator

Ascension is seeking a Registered Nurse Patient Navigator to guide patients through complex care journeys within our compassionate, mission-driven healthcare system. In this role, you will coordinate appointments, educate patients and families, and facilitate communication among multidisciplinary providers across hospitals, clinics, and community settings. You will assess clinical needs, remove barriers to care, support care transitions, and promote adherence to treatment plans. The Navigator advocates for patient-centered, culturally competent care while contributing to quality improvement, population health initiatives, and ongoing innovation in healthcare delivery.
