The compensation offered to the candidate selected for the position will depend on several factors, including the candidate's educational background, skills and professional experience.
Requirements
- APN License
- RN License
- NP Certification
- CDS and DEA
- ACLS and BLS
- 2+ years of experience in geriatrics
Shift:
- M-F (8:30a-4:30p)
- 4 clinic and consult days
- 1 day follow up home visits for high risk patients recently discharged from the hospital
Essential Functions
- Provide comprehensive primary care with a focus on geriatric needs and chronic disease management
- See a manageable caseload (about 8-12 patients per day), allowing time for thorough visits and care coordination
- Work collaboratively with the team to support complex patients and promote healthy aging
- Participate in hospital home visit initiative to help reduce readmissions
