Medical Social Worker Hospice Part-Time/PRN

Medi Home Hospice, a proud member of the Medical Services of America family, is currently seeking an experienced Part-Time/PRN Medical Social Worker for our Hospice Patients in Greensburg and Westmoreland, PA. All the relevant skills, qualifications and experience that a successful applicant will need are listed in the following description. · Provides social work services in accordance with the physician’s plan of treatment. · Works with the interdisciplinary team directly, or through coordination of team members, to provide appropriate assessment and support patients and families. · Provides assessment for normal and complicated grief and provides bereavement counseling as appropriate. · Serves as a liaison with other community resources.
