This outpatient clinic requires a board-certified Internal Medicine physician to deliver primary care services to a chronically ill adult patient population while the facility's full-time clinician is on leave. The facility operates on a structured day shift schedule and maintains a multidisciplinary support team to facilitate coordinated and quality care delivery. Clinical workflows are organized to address the ongoing medical needs of the patient population served.

The Internal Medicine physician will provide outpatient primary care services to an adult patient population with chronic illnesses at an on-site clinic, working Monday through Friday from 8:00 AM to 5:00 PM during day shift hours. The expected caseload is 17-20 patients per day, with clinical responsibilities centered on the evaluation and management of chronic medical conditions. The clinician will utilize EPIC EMR for all clinical documentation and will work collaboratively with a multidisciplinary support team that includes Behavioral Health staff, 2 PharmDs, and 2 FNPs. No call coverage is required for this assignment.
