Job Description

About the RoleDimer Health is looking for a healthcare operator who can build trusted relationships with our health-system partners, lead successful program implementations, and help those partnerships grow over time. This is a field-based, full-time role covering our New Jersey and Pennsylvania health-system partnerships, with regular on-site time at hospitals and health systems. You’ll be our partners’ primary point of contact from the moment a contract is signed. You’ll lead implementation, become the trusted clinical resource on the ground, and grow the relationship over time.Reporting to the Chief Commercial Officer, this role sits where clinical operations, customer success, implementation, and commercial growth meet. We’re looking for someone with real clinical and operational credibility who can develop commercial instincts. We are not looking for a salesperson who is trying to learn the clinical side.

Do not pass up this chance, apply quickly if your experience and skills match what is in the following description.
What You’ll Do
Build trusted relationships – Earn the trust of hospital leaders and stakeholders in case management, nursing, care management, quality, and operations, across every level of each health system.Be present on-site – Spend significant time at partner hospitals so you become a trusted resource there, and not only a point of contact.Lead implementation and go-live – Turn signed contracts into workflows, protocols, and an operating model that fit each partner’s environment and systems.Coordinate across teams – Bring together clinical, staffing, operations, and business development stakeholders around clear operating rhythms that shorten the time from contract signature to launch.Anticipate barriers – Spot implementation risks before they become problems, and make sure partners understand what operational success looks like.Own the partnership after launch – Monitor adoption, engagement, workflow performance, and satisfaction, and resolve issues before they escalate.Track performance – Measure results against clinical quality, utilization, and contract targets.Grow each account – Find and develop expansion opportunities and know when to involve commercial leadership. Examples include extending a workflow that worked at one facility to another, reaching an adjacent population or service line, or bringing in a new stakeholder.Connect the partner and Dimer – Know when to bring in clinical leadership, operations, product, or executive leadership to solve a problem.

What You’ll BringRequiredA real, lived understanding of hospital discharge, discharge disposition, case management, care coordination, transitional care, post-acute care, readmissions, and patient handoffs.Enough hospital workflow fluency to quickly understand how a health system’s case management, nursing, physician, social work, utilization management, and quality teams interact.A track record of building trust with both frontline clinicians and executives, without coming across as a traditional salesperson.Experience taking an ambiguous initiative and giving it structure: running kickoffs, building implementation plans, coordinating stakeholders, and driving adoption.A commercial mindset. You notice unmet needs and expansion opportunities, and you understand the difference between solving a problem and selling a product. You don’t need to have carried a sales quota.Willingness and ability to be on-site with partners most of the time.
PreferredDirect experience implementing a healthcare technology, clinical program, or care-management initiative.A move from a clinical or operational role into a customer-facing one, such as clinical liaison, provider relations, implementation, customer success, or account management.Existing relationships within New Jersey hospitals or health systems.
Strong candidates come from many backgrounds and titles. These include Nurse Manager, Case Management Manager, Director of Case Management, Care Management Manager, Transitional Care Manager, Clinical Program Manager, Clinical Liaison, Clinical Account Manager, Client/Customer Success Manager, Implementation Manager, Provider Relations Manager, and Clinical Partnerships roles.
