You will triage level of care, coordinate urgent referrals, and deliver brief, evidence-based interventions in close collaboration with psychiatrists, nurses, and primary care providers. The position operates in a hybrid environment with both on-site and telehealth services, requiring strong communication, documentation, and teamwork skills. You will contribute to continuous improvement initiatives that enhance access, patient outcomes, and integrated care.
