Job Description

Home Care Coordinator - Registered Nurse 
Pay Range: $90,000–$102,000

Responsibilities:

1. Lead assessments, care planning, and skilled nursing for home care clients
2. Build relationships with clients, families, and physicians
3. Mentor and empower direct care staff, ensuring compliance with regulations

Qualifications:

• Clinical nursing expertise and home care leadership experience
• Celebration of aging and ability to build trust
• Strong organizational skills and regulatory knowledge
