Nurse Navigator Cardiac

MedStar Health seeks a Nurse Navigator Cardiac to guide patients and families through the cardiac care journey. In this RN role, you will coordinate multidisciplinary services, streamline appointments, and support safe transitions between inpatient, outpatient, and home settings. You will provide education on cardiac conditions, procedures, and lifestyle changes, while monitoring clinical status and addressing barriers to care. Working within a collaborative, mission-driven team, you will use evidence-based practices, EHR tools, and quality metrics to improve outcomes for a diverse regional patient population.
