In this role, you will evaluate patients, develop and implement individualized treatment plans, and guide patients through therapeutic interventions to restore function, mobility, and independence. You will collaborate closely with physicians, nurses, and an interdisciplinary rehab team to ensure safe, patient-centered care. Responsibilities include accurate, timely documentation, patient and family education, and participation in care conferences and quality initiatives.
