What You’ll Do - Provide skilled nursing care in accordance with physician orders and organizational policies

  • Perform comprehensive assessments and develop individualized care plans
  • Collaborate with physicians, therapists, social workers, and home health aides
  • Supervise and support LPNs and Home Health Aides
  • Ensure compliance with regulatory standards and contribute to quality improvement initiatives
  • Promote patient satisfaction and positive health outcomes
What You Bring - Current RN license (BSN preferred)
  • Minimum 1 year of nursing experience (required in California)
  • Home healthcare experience preferred
  • Valid driver’s license and reliable transportation
  • Strong computer skills and a commitment to ethical, mission-driven care

About Saint Agnes Home Health and Hospice (California) Saint Agnes Home Health and Hospice is a member of Trinity Health At Home, a national home care, hospice, and palliative care organization serving comnities throughout eleven states. As a faith-based, not-for-profit agency, we serve patients and families in the comfort of home, offering skilled nursing, therapy (physical, occupational, speech), and medical social work. We are Medicare-certified and accredited by The Joint Commission. Physical & Mental Requirements - Frequently lift, stand, and walk (35% of time)
  • Occasionally bend, stoop, twist, and assist with patient positioning (10%)
  • Independently lift up to 35 lbs
  • Perform detailed tasks with frequent interruptions and shifting priorities
  • Adapt to changing work environments and travel to various work locations
