Registered Nurse, Hospice Home Care

VNS Health is seeking a compassionate Registered Nurse to provide hospice home care services to patients in the Bronx. In this role, you will deliver skilled nursing care, symptom management, and end-of-life support in patients’ homes. You will develop and implement individualized care plans, coordinate with physicians, social workers, and other team members, and educate families on comfort measures and disease progression. The RN will conduct comprehensive assessments, ensure accurate documentation, uphold regulatory standards, and support patients and caregivers with sensitivity, respect, and cultural competence.
