Job Description

Physical Therapist (PT) Pathways PACE | Greeley, Colorado Full-Time | Monday-Friday | 8:00 AM-5:00 PM Salary: $80,308 - $87,900 annually Pathways PACE is excited to have a brand-new Greeley location, and we're looking for a Physical Therapist who wants to help shape the future of care for older adults in Northern Colorado. This isn't just another PT position. It's an opportunity to join a founding clinical team, build strong interdisciplinary relationships, and make a lasting impact on the lives of older adults by helping them remain independent and living safely in their own homes. If you're looking for meaningful patient relationships, a collaborative team, predictable weekday hours, and the chance to influence a new program, we'd love to meet you. Why Pathways PACE? At Pathways PACE, we believe exceptional care begins with exceptional people. We invest in our team by offering:

  • Monday-Friday schedule (8:00 AM-5:00 PM) — no weekends or major holidays
  • Employer-paid medical premiums (over 90% on select plans)
  • Health Savings Account with employer contributions ($1,000 individual / $2,000 family)
  • 18 days of PTO in your first year
  • Seven paid holidays plus one floating holiday
  • 403(b) retirement plan with employer match
  • Company-paid life and AD&D insurance
  • Continuing education reimbursement
  • Clinical career ladder opportunities
  • Certification pay
  • Leadership development programs
  • Early wage access
  • Legal and identity protection benefits
  • Generous employee referral bonuses
  • Mileage reimbursement at 62.5¢ per mile—one of the highest reimbursement rates in the industry
What Makes This Role Different As a Physical Therapist with Pathways PACE, you'll work as a valued member of an interdisciplinary team that truly collaborates to help participants remain healthy, independent, and living in the community. You'll have the opportunity to:
  • Build lasting relationships with participants and their families.
  • Evaluate functional abilities and develop individualized treatment plans.
  • Recommend home modifications and adaptive equipment that improve safety and independence.
  • Collaborate daily with physicians, nurses, physical therapists, social workers, and other disciplines.
  • Help participants maintain meaningful daily activities and improve their quality of life.
  • Play an important role in launching a new PACE program and helping establish best practices.
Instead of seeing patients for just a few visits, you'll become part of a team that follows participants over time, allowing you to truly see the impact of your care. Qualifications We're looking for someone who is passionate about helping older adults thrive. Required:
  • Graduate of an accredited entry level program in Physical Therapy
  • Active Colorado Physical Therapist license
  • Current CPR certification
  • Valid Colorado driver's license and proof of insurance
  • One year working with elderly population, home care or geriatric experience
Preferred:
  • Ability to work effectively with culturally, economically, and educationally diverse populations
  • Experience working with interdisciplinary teams
  • An interest in preventative, community-based care
Why PACE? PACE (Program of All-Inclusive Care for the Elderly) is designed to help older adults live safely and independently in their own homes for as long as possible. As a Physical Therapist, you'll have the time, resources, and interdisciplinary support to provide truly patient-centered care—not productivity-driven care. If you're looking for a career where your expertise directly improves lives every day while helping launch an exciting new program in Greeley, we'd love to hear from you. Apply today and help us build something extraordinary.
