Duties

Develops, implements, and evaluates community health promotion and disease prevention programs based on identified health needs, available resources, and program effectiveness.

Coordinates community health assessments and information-gathering activities, including surveys, focus groups, interviews, and other methods used to identify health priorities and evaluate outreach efforts.

Develops and delivers health education and outreach materials and activities, including presentations, reports, posters, pamphlets, media products, and community feedback sessions.

Provides training, consultation, and technical assistance to community health workers, clinical staff, educators, tribal programs, and other partners supporting health promotion and community development initiatives.

Establishes and maintains collaborative partnerships with community, Tribal, Federal, State, school, healthcare, and other organizations and facilitates community forums and stakeholder input to identify and address local health priorities.
