Job SummaryTrinity Home Health is a boutique home health agency dedicated to providing high-quality, patient-centered care in the comfort of our patients’ homes. We are currently seeking an experienced Registered Nurse (RN) to join our team on a full-time or part-time basis in the San Luis Obispo North County region.The Home Health RN will be responsible for delivering skilled nursing care, coordinating patient care plans, and collaborating with an interdisciplinary team to ensure excellent outcomes. xzbcwsv This role is ideal for a self-motivated nurse who values autonomy, flexibility, and meaningful patient relationships.
Key ResponsibilitiesPerform skilled nursing visits in patients’ homes in accordance with physician orders and plan of careComplete comprehensive assessments, ongoing evaluations, and accurate clinical documentationAdminister medications, wound care, injections, and other skilled nursing treatmentsEducate patients and caregivers on disease processes, medications, and home care techniquesCoordinate care with physicians, therapists, and other members of the care teamMonitor patient progress and promptly report changes in conditionEnsure compliance with home health regulations, agency policies, and standards of careQualificationsActive California Registered Nurse (RN) licenseMinimum of 1 year of home health nursing experience requiredCurrent CPR certificationStrong clinical assessment and critical-thinking skillsExcellent communication and documentation abilitiesAbility to work independently and manage time effectivelyReliable transportation and valid driver’s licenseFamiliarity with OASIS and EMR systems required
