A growing behavioral health organization is seeking an experienced Psychiatric Nurse Practitioner to join its Assertive Community Treatment (ACT) Team in Brooklyn, NY.
This is a field-based opportunity for a provider passionate about community mental health and delivering psychiatric care directly to individuals in their homes and other community settings. The ideal candidate has experience treating patients with serious mental health conditions and a strong understanding of the ACT model.
Responsibilities:Conduct psychiatric evaluations, medication management, and ongoing treatmentProvide in-home and community-based psychiatric care throughout BrooklynCollaborate with an interdisciplinary ACT Team to coordinate comprehensive patient careSupport individuals with serious mental health conditions and complex behavioral health needsDevelop and manage treatment plans to promote stability, recovery, and continuity of careWork collaboratively with other clinical professionals to help patients access appropriate services and remain engaged in treatment
Qualifications:Active New York State Psychiatric Nurse Practitioner license and certificationAt least 1 year of experience as a Psychiatric Nurse PractitionerSolid understanding of the Assertive Community Treatment (ACT) modelComfortable conducting field visits and traveling throughout BrooklynStrong clinical assessment, communication, and interdisciplinary collaboration skills
Compensation: Salary up to $170,000, depending on experience. xzbcwsv
