Bereavement Coordinator

Shift and Status Bereavement Coordinator, Inpatient Hospice Part time, Monday, Tuesday, Wednesday, and Friday 8:00 a.m. to 4:30 p.m. Hybrid Job Summary Develops and implements the bereavement program. Oversees bereavement plans for each patient and family. Provides group support and individual counseling to bereaved clients per bereavement plan of care. Essential Duties Assesses bereavement needs and provides bereavement follow-up to each bereaved referred to the Bereavement Program. Plans, develops, and implements Bereavement Support Groups and educational workshops on specific grief issues. Plans Memorial Service(s). Participates in Team meetings.
