- Perform comprehensive PT evaluations and develop a plan of care for SNF residents.
- Implement evidence-based interventions: therapeutic exercise, gait training, balance, ROM, and pain management.
- Reassess progress regularly, adjust treatment plans, and document outcomes in resident records.
- Collaborate with nursing and rehab teams; contribute to discharge planning and caregiver education.
- Ensure patient safety, maintain HIPAA-compliant documentation, and comply with facility policies and regulations.
- Active physical therapy license in good standing in the practice state or eligibility for licensure transfer per assignment policies.
- Strong clinical assessment, documentation, and communication skills.
- Ability to work on-site in a skilled nursing facility setting.
- CPR/BLS certification.
- Ability to travel and commit to a 13-week assignment.
