Position OverviewJoin a mission-driven community health organization providing comprehensive primary care to diverse patient populations throughout the region. We are seeking Family Practice Nurse Practitioners and Physician Assistants to provide full-scope primary care across established community clinics.
Providers will be assigned a primary clinic location, with the opportunity—and occasional need—to float to nearby sites based on patient volume and operational needs. Most providers work at one location per day, offering regional flexibility without constant same-day site changes.
This is an excellent opportunity for both experienced providers and new graduates looking to build broad clinical experience in a high-volume community health setting.
Key ResponsibilitiesProvide comprehensive family practice and primary care to a diverse community patient populationConduct evaluations, diagnose and treat acute and chronic conditions, and develop individualized treatment plansProvide preventive care, health education, and ongoing disease managementSupport patient needs across a broad range of primary care concerns rather than a narrow specialty populationMaintain accurate and timely clinical documentationCollaborate with multidisciplinary care teams to promote quality outcomesFloat to nearby clinic locations as needed based on patient volume and organizational needsRamp into a standard clinic schedule and maintain productivity expectations of approximately 3,000 encounters annually

Each provider will have an assigned primary location. Nearby floating will be discussed during the interview process and coordinated in advance. Many clinic locations are close to one another, making occasional floating manageable. xzbcwsv
