Care Coordination Registered Nurse (RN)

Hartford Healthcare seeks a compassionate Care Coordinator RN to support patients across our Connecticut network. In this role, you will coordinate care plans, manage transitions of care, and collaborate with physicians, nurses, and community providers to ensure safe, high-quality outcomes. You will conduct patient assessments, provide education on conditions and treatments, and monitor progress to reduce readmissions. Working with our interdisciplinary team, you’ll advocate for patients, connect them to appropriate resources, and help streamline services.
