• Provide skilled nursing care to home health patients, including assessment and ongoing monitoring
• Perform OASIS visits and data collection as part of patient care planning and regulatory compliance
• Develop, implement, and adjust individualized care plans in collaboration with the interprofessional team
• Administer medications and deliver appropriate clinical interventions within scope of practice
• Educate patients and caregivers on disease management, safety, and treatment plans
• Document all care and observations accurately in the patient record; ensure compliance with policies and regulations
• Coordinate with other providers and services to ensure smooth transitions and timely follow-up
• No on-call requirements; participate in a weekend rotation as scheduled

• Active Registered Nurse license in the State of Maine
• BLS certification
• Minimum of 1 year of Home Health Care experience
• Ability to perform OASIS visits
• Weekend rotation required (approximately 2–3 weekend days per month)
• Able to work in a home health setting with appropriate safety and infection control practices
