Care Manager - Middlesex County - Part time - Hourly rate $43.00 - The areas highest quality behavioral healthcare providing care management services to clients. Responsibilities include assessing and assisting with clients needs and coordinating care in a warm and compassionate environment, 8 +/- hours per week, 9:00am - 5:00pm Monday - Friday - The Care Manager will follow through and assist clients with their plan of care, and coordinate care services ensuring that those we serve receive the highest quality of service in a caring and compassionate atmosphere. MINIMUM QUALIFICATIONS

• Bachelor's degree required
• Ability to read, write, understand, and communicate effectively in English
• Drivers license in good standing
• Possess insured vehicle to travel to and from clients homes
• Ability to deal with others in a diplomatic, tactful and confidential manner
• Ability to maintain composure under stress in dealing with residents, staff, and families
• Flu vaccine required during flu season
