Licensed Practical Nurse

OneHome is seeking a Licensed Practical Nurse to deliver compassionate, home-based care for patients receiving home health, infusion therapy, and durable medical equipment services. The LPN will provide direct clinical care in the home, including assessments, medication administration, wound care, and monitoring of chronic conditions under RN or physician direction. You’ll coordinate with an interdisciplinary team, document accurately in electronic systems, educate patients and caregivers, and escalate changes in condition. This mission-driven role blends hands-on nursing with technology-enabled care to improve outcomes and support value-based, integrated home health at scale.
