Job Summary

Seeking a Physician, Nurse Practitioner, or Physician Assistant to provide wound care services in skilled nursing and long-term care facilities. No prior wound care experience is required; training is provided.

Responsibilities

• Conduct patient assessments and weekly facility rounds.
• Diagnose, treat, and monitor wounds.
• Perform debridement when clinically indicated.
• Collaborate with facility staff and coordinate referrals.
• Maintain accurate patient records using EMR.

Requirements

• MD, DO, NP/CRNP, or PA qualification.
• Applicable board certification or eligibility.
• Active, unrestricted New York State license.
• DEA registration, as applicable.
• Valid driver's license and strong documentation skills.
