Schedule: Monday–Friday, 8:00 AM–5:00 PM
Travel: 50–75% field travel

Job Summary:
We are seeking a Case Management Field Analyst to support Long-Term Services & Supports (LTSS) members. This field-based role manages a full caseload, conducts face-to-face member visits, and provides care coordination, education, and support to help achieve appropriate healthcare outcomes.

Key Responsibilities:

Conduct comprehensive member assessments and identify healthcare/service needs.
Coordinate and monitor care plans and member progress.
Conduct required quarterly in-person visits.
Collaborate with case managers, supervisors, and healthcare professionals to address barriers to care.
Provide coaching, education, and support to improve member engagement and health outcomes.
Monitoring, Evaluation and Documentation of Care
Document activities in accordance with applicable policies and regulatory requirements.

Requirements:

Minimum 2 years of case management experience.
Valid Illinois Driver’s License and reliable transportation.
Willingness to travel up to 50-75% for face-to-face member visits.
Experience with Microsoft Office and Electronic Health Records (EHR).
Bachelor’s degree required
